How do I get an Arizona Medical Marijuana Program Registry Identification Card?
If you are applying for an Arizona medical marijuana card for the first time, you’ll need a recommendation from a physician based on a qualifying condition. However, not all doctors are willing to recommend medical marijuana. If you have a qualifying condition (see list below), but your general practitioner is not willing to recommend medical marijuana, we suggest making an appointment at a medical marijuana clinic where you will be evaluated. If the physician approves your request for an MMJ recommendation, they will take care of all the paperwork at the time of your visit.
If your general practitioner is willing to recommend medical marijuana for your qualifying condition, the Arizona Department of Health Services provides online instructions and a clear checklist for you to submit an application online.
What are the qualifying conditions for a medical marijuana card in Arizona?
Qualifying conditions to become a medical marijuana patient in Arizona include:
My doctor recommended medical marijuana for my condition. Can I purchase marijuana from your dispensary with a doctor’s note?

Why do I need a medical marijuana card to purchase marijuana in Arizona?
In the state of Arizona, recreational marijuana use is still illegal because the state failed to legalize for recreational use in November 2016. This means that you must be an Arizona medical marijuana cardholder to legally purchase medical marijuana from a dispensary.

There is an initial fee of $150 for the identification card for a qualifying patient. Some qualifying patients may be eligible to pay $75 for first-time applicants and card renewals if they currently participate in the Supplemental Nutrition Assistance Program (SNAP). Physician evaluations range in cost. The average annual cost for an Arizona medical marijuana card is between $275 and $330.
I was approved for an AZ MMJ card, but I haven’t received my ID card in the mail yet. Can I purchase marijuana with my approval paperwork?
